Steere House has the rehabilitation program for you or a loved one following a stroke, heart attack, accident, hospitalization for a chronic illness or elective surgery.

Steere House has expanded the current program for short lengths of stay.  This enhanced program for post acute hospital care, called transitional care, includes the establishment of a separate and distinct nursing unit called the Transitional Care Unit (TCU).  This specialized short stay program provides cardiac reconditioning and orthopedic rehabilitation as well as rehabilitation following hospitalization for chronic illnesses for patients who will ultimately return home or be discharged to other appropriate settings.

The Steere House team of professionals including primary care doctors, specialists, physical, occupational and speech therapists, registered nurses educated in cardiac reconditioning and certified nursing assistants trained in rehabilitation have the following goals:

• To return patients to their optimum level of functioning.
• To coordinate rehabilitation and medical services to provide continuity of care upon discharge.
• To provide a smooth transition back into the community.
• To have each patient leave feeling good, confident in their abilities and well cared for while at Steere House.
